The technology has been widely regarded for its ability to test the association of symptoms with GER, regardless of the acid content. By convention, a GER event occurring within the pre-symptom time window (eg, 2 minutes) is considered to be temporally associated with the symptom. Hence, symptom association indices (SAI) (the symptom index, the symptom sensitivity index, and symptom association probability index) can be calculated as an objective quantification of the temporal relationship between GER and symptoms. SAIs are calculated by the system software and rarely by the system operator (technician or clinician). A positive (or negative) SAI can have a major impact on the decision-making process as the clinician develops a strategy for clinical management of the patient. Combined pH-MII for assessment of GER symptoms in infants and children has been used at our institution since the early 2000s. Consequently, some important observations regarding the manner in which pH-impedance system software establishes temporal relationships and calculates SAIs have been made. One important observation is reported with regard to the Medical Measurement Systems pH-MII system (Netherlands), in particular. For symptom-GER association, if the “onset” of the GER event occurs within a selected time window (we use 120s), the software marks a positive symptom-GER correlation. Otherwise, it marks a negative correlation. Acid GER (AGER) episodes that overlap the pre-symptom time window but are not initiated within the window will not be temporally associated with the symptom. Figure shows a bi-phasic AGER episode during which the majority of volume clearance and all of chemical clearance overlaps with a 2-minute window. While likely associated with the symptom (in this case, abdominal pain), this AGER episode is not registered by either the pH probe or the impedance catheter as a positive association because neither metric onsets, pH and impedance (2:28:31), are initiated within the pre-symptom time window (2:28:37-2:30:37). Impedance pH Monitoring: Intra-observer and Inter-observer Agreement and Usefulness of a Rapid Analysis of Symptom Reflux Association

BACKGROUND/AIMS Symptom reflux association analysis is especially helpful for evaluation and management of proton pump inhibitor (PPI) re-fractory patients. An accurate calculation requires manual editing of 24-hour multichannel intraluminal impedance-pH (MII-pH) tracings after automatic analysis. Intra- and inter-observer agreement as well as reliability of rapid editing confined to the time a...

Effectiveness of Stress Management Training in Reducing Symptoms of Gastroesophageal Reflux Disease among Women

Background & Aims: Considering the important role of stress in intensifying gastroesophageal reflux disease (GERD) symptoms, the aim of the present study was to determine the effectiveness of stress management training in reducing symptoms in patients with GERD. Methods: In the present semi-experimental study, a pre-test post-test design with control group was used. From among the patients who ...

Pulmonary Scintiscan Findings in Sulfur Mustard Injured Patients Suspected for Gastroesophageal Reflux; a Descriptive Cross-Sectional Study

Background:Gastroesophageal reflux disease (GERD) prevalence in patients with sulfur mustard (SM)-induced bronchiolitis obliterans (BO) is higher than exposed cases with mild lung injuries. In this study, we aimed to evaluate the prevalence of microaspirations using nuclear scintiscan among BO patients with SM exposure. Methods: This was a prospective cross-sectional study conducted on patient...

Role of PH Monitoring in Laryngopharyngeal Reflux Patients with Voice Disorders

Introduction: Laryngopharyngeal reflux (LPR) is considered an important cause of voice disorder. We aimed to determine the frequency of LPR in patients with voice disorder and the association between Koufman Reflux Symptom Index (RSI), Reflux Finding Score (RFS), gastroesophageal reflux disease (GERD), and proximal acid reflux in these patients. Materials and Methods: We performed a prospective...

The Effects of Pantoprazole on the Treatment of Palpitation in Patients with Gastro esophageal Reflux Disease (GERD): A Case Series

Introduction: Atrial arrhythmia is a common complication in patients with gastro esophageal reflux disease (GERD). The treatment of palpitation is relatively problematic in these patients, especially if there is not enough evidence of cardiovascular or systemic diseases. The esophagus is in close proximity to the left atrium posterior wall. Hypothetically, locally released cytokines from esopha...
